AVATAR provides the following services:

• SKILLED NURSING
  Registered Nurses and Licensed Vocational Nurses visit the patients to help the Doctors establish a Plan of Care and monitor the patients progress.

   

• HOME HEALTH AIDES
  Certified Nurse’s Assistants visit the patients regularly to monitor their progress and help with basic living necessities.

   

• PRIMARY HOME CARE
  Trained providers help the patients with their everyday needs, such as: cleaning, cooking, shopping, bathing, etc.

   

• RESPITE CARE
  When the caretaker, usually a relative, needs a few days rest, our Primary Care and/or Home health aids, take over their duties until they can return.

   

• CASE MANAGEMENT
  A trained professional coordinates and overviews the Patient’s Plan of Care, to insure that the patient’s needs are being met.

   

• ANODYNE THERAPY
  A Certified Anodyne Therapy Provider treats the patient with  the latest technology available. This therapeutic process reduces pain, improves circulation, promotes wound healing and helps in many other areas.

   

• HOME MEDICAL EQUIPMENT
  AVATAR will assist the patient obtaining medical equipment this could include; canes, wheel chairs, beds, mattresses, diabetic supplies, oxygen, etc….

   

• PHYSICAL THERAPY
  A Licensed and Registered Physical Therapist evaluates the patient and recommends a treatment plan, if needed. This is followed by scheduled visits for the required treatment.

   

• SPEECH LANGUAGE PATHOLOGY
  A Licensed and Registered Speech Therapist evaluates and treats the patient, if needed, to help him/her recover or improve his speaking abilities.

   

• OCCUPATIONAL THERAPY
  A Licensed and Registered Occupational Therapist evaluates and treats the patient, if needed, to improve the patient’s motor skills.

   

• MEDICAL SOCIAL WORK
  A Licensed Social Worker evaluates patient’s needs and recommends other services that will improve the patient’s quality of life.
   

• TRANSPORTATION
  AVATAR helps patients obtain transportation to fulfill their medical requirements.
